Parents Urged to Begin Back-to-School Immunizations

The Orange County Health Department’s (OCHD) Immunization Program is urging parents to begin preparing their children now for the upcoming school year.  Parents can visit their children’s pediatrician or medical provider of choice to have their children vaccinated and avoid the back-to-school rush and long lines. Families that do not have a medical home where their children can be seen by a physician and obtain their immunizations can refer to the link below for a Resource List of Children’s Medical Providers or visit the Orange County Health Department’s Immunization Clinic.

The Orange County Health Department Immunization Program provides back-to-school immunizations Monday through Friday from 7:30am to 3:00pm at its Central Office located at 832 West Central Boulevard.  Parents are urged to arrive early as tickets are usually given out by 10:00am during the months of July and August. In addition to weekday hours, the health department’s Immunization Program is providing back-to-school immunizations on Saturday, July 30, 2011 from 7:30am – 2:00pm at the Central Office.

Due to the high demand for immunizations, services are provided on a first come, first served, walk-in basis. Children must be accompanied by an adult family member or legal guardian to receive immunizations.  If not, a notarized permission form, signed by the parent or legal guardian, must be presented before services are rendered.  A copy of each child’s immunization record and government-issued, valid photo identification of the adult relative or legal guardian are required.

A certified DH 680 immunization form is required for school entry and is provided free once your child has received all of their vaccinations. If a client only needs a DH 680 form there is an administrative fee.  You can also obtain this form at your child’s doctor’s office and the OCHD immunization clinic or medical records departments if your child is already up-to-date on their immunizations.  This year, the Florida SHOTS installed a new release that allows healthcare providers to issue parent retrieval PINs so they may get their child’s new or updated DH 680 form from the Florida SHOTS website.  The OCHD Immunization’s program automatically provides the client with their PIN number when a certified DH680 form is issued or you may ask your healthcare provider if he can issue the PIN.
