Neither guards nor supervisors called 911 for help before an 18-year-old teen died ten days ago, at a West Palm Beach lockup, Florida Department of Juvenile Justice (DJJ) administrators confirmed on Monday, the Miami Herald reports.

Eric Perez died at a West Palm Beach juvenile detention center on July 10, several hours after he appealed for help as his pain intensified and he began the throw up.

While the investigation is ongoing into the cause of Perez’ death, DJJ said that the facilities top administrator, Superintendent Anthony Flowers was among four employees suspended since the incident.  Two other lockup workers, a guard and a supervisor, have been fired.

“While the cause of death is yet unknown, it is clear that staff at the facility during the crisis did not contact 911 in accordance with DJJ policies and training,’’ Samadhi Jones, a DJJ spokeswoman in Tallahassee, said in a statement Monday.
