Seven Florida practitioners were suspected for serious violations relating to the commission of crimes under the Emergency Suspension Order (ESO), Florida Department of Health (DOH) announced on Wednesday.
Over the past week the following ESO’s were filed:
- Lisa Marie Dwualt, L.P.N, Polk City. Dwualt deemed unable to practice nursing with reasonable skill and safety to patients by reason of illness or use of alcohol, drugs, narcotics, or chemicals or as a result of a mental or physical condition.
- Unitsia A. Jenkins, C.N.A., Jacksonville. Jenkins entered a plea or nolo contendere to one count of sale or delivery of cocaine.
- Laniel McClellan, R.N., St. Petersburg. McClellan entered a guilty plea to one count of possession of cocaine.
- Jamie Lynn Phillips, C.N.A., Naples. Phillips entered a plea of nolo contendere to one count of fraudulent use of credit cards.
- Cara L. Taylor, L.P.N., Spring Hill. Taylor entered a guilty plea to one count of organized fraud.
- Rachelle Rae Stuart, C.N.A., Salt Springs. Stuart engaged in unprofessional conduct on her drug screen by testing positive for any drugs on her drug screen without a prescription or legitimate medical reason.
- April Nicole Crumpton, C.N.A., Palatka. Crumpton engaged in unprofessional conduct on her drug screen by testing positive for any drugs on her drug screen without a prescription or legitimate medical reason.
“The Florida Department of Health remains committed to identify and investigate people illegally practicing medicine or revealing unbecoming conduct,” stated DOH Surgeon General Dr. Frank Farmer. “By using the appropriate disciplinary tools, such as Emergency Suspension Orders, we can ensure we protect public health.”
