On May 6th, 7th and 8th, Community Coordinated Care for Children (4C) will host an exclusive opportunity for parents to apply to 4C-Early Head Start. The three-day event, scheduled for Tuesday through Thursday, will be held at the Dr. Smith Neighborhood Center at 1723 Bruton Boulevard in Orlando, Florida, from 8:30 a.m. to 2:30 p.m.
4C, the non-profit corporation which provides educational and childcare resources for Florida families, is hosting this application fair to ensure that eligible families have an opportunity to apply for an Early Head Start program, which will provide a good foundation for elementary, middle and high school education.
“We invite parents of children ages four and under to come and meet with us to explore what Early Head Start has to offer,” said Pat Frank, president and CEO of 4C. “The program provides an opportunity for children to learn the basics of reading, writing and math, ensuring that they are ready for school when it’s time to start kindergarten.”
Parents who attend the 4C Early Head Start Application Fair are asked to please bring the following documents to the appointment:
- Parent ID
- Proof of Age:
- Birth Certificate
- Passport
- Income Eligibility:
- All income for the past 12 months for all family members
- Proof of Address
- Utility bill
- Lease
- Additional forms to include…
- Disability—copy of the IEP/IFSP or Private Evaluation
- Family Crisis/At Risk
- Domestic Violence (documented by Agency)
- Single Parent – Incarcerated or Military Deployed (documented by Agency)
- Alcohol, Drug Abuse, or Mental Illness (documented by Agency)
- Agency Referral
- DCF/Court Referral/EHS/Homelessness/OCPS Diagnostic
